All single and/or unaccompanied Soldiers will need to fill out the 5960 form for housing. Please fill out the following on the form:- Boxes 1, 2, 3, 4-x on change
- Box 5, 6, 8-x on single
- Skip to box 10 and only fill out your current address in the middle box then sign box 13, date box 14 and fax to our office at (785) 717-2209
